In short, Isaura, if you knew how well I love you, you wouldn't make so little of me.
The plea drops to something almost sincere: 'se você soubesse quanto bem te quero,' if you knew how I love you. 'Fazer pouco caso' — to make little of, to disregard — is the wound he feels, the slight of being ranked beneath his own vanity. Even here the appeal is about him, his hurt, his worth; Isaura's own wishes never enter it.
