— Como?!— exclamou ella com um accento que exprimia a um tempo altiva indignação e o mais entranhado desgosto.
— What?! she exclaimed, in a tone that expressed at once haughty indignation and the most deep-rooted grief.
Malvina's calm shatters. Her 'Como?!' carries two things 'a um tempo,' at the same time — 'altiva indignação,' proud outrage at the injustice, and 'entranhado desgosto,' grief rooted in the entrails, the deepest hurt. The doubling is the truth of her: she is at once the wronged wife who will not be trifled with and the loving woman whose heart is breaking.
